This is the official electronic entry form for the Tri-State Better Business Bureau's Torch Award. The following are the steps to be taken for on-line submission.
You will be able to review and revise your responses if you wish.
You may upload Word documents or PDF format files, containing your answers versus typing them online if you like. Having these saved on your computer first will help.
After you are satisfied with your responses, click "SUBMIT Application" to send in your completed application. At this point, no further edits or review are possible.
Question 3 is optional and you may again upload a file with a response if you like.
Up to 3 additional/supplemental documents will be accepted for this question, however you will not be able to upload the supplements until the end of the application process.
After submitting your application you will not be able to re-enter the application form.
